Speak with, or visit, your doctor to request new prescription(s) for your diabetes testing supplies and medications. Contact us for assistance or more information. 2. Complete our enrollment questionnaire. Enroll today! 3. Mail us your prescription(s), the completed enrollment questionnaire, and any applicable out-of-pocket costs (i.e., copay, deductible or coinsurance for your prescription and supply order - BLN can assist you with determining this amount) for which you are responsible to: Better Living Now, Inc.
